Nocturnal penis tumescence (abbreviated as NPT ) is a spontaneous erection of the penis during sleep or waking. All men with no physiological erectile dysfunction experience penis enlargement at night, usually three to five times at night, usually during REM sleep. NPT is believed to contribute to penis health.

The presence and predictability of nocturnal tumescence is used by sexual health practitioners to ascertain whether a given erectile dysfunction (ED) case comes from psychology or physiology. A patient who presents E.D. equipped with elastic device to be worn around his dick while sleeping; the device detects thickness changes and passes information to the computer for later analysis. If nescturnal tumescence is detected, then E.D. allegedly due to psychosomatic diseases such as sexual anxiety; if not, then allegedly caused by physiological causes.

Mechanism
The cause of NPT is not known with certainty. Bancroft (2005) hypothesizes that noradrenergic neurons of the seruleus locus are inhibition for penile erection, and that the cessation of their expenditure during REM sleep may allow testosterone-related action to manifest as NPT.
Evidence that supports the possibility that a full bladder can stimulate an erection has existed for some time and is characterized as an 'erection reflex'. The nerves that control a man's ability to have a reflex erection are located in the sacral nerve (S2-S4) of the spinal cord. A full bladder is known to slightly stimulate nerves in the same region.
The possibility of a full bladder causing an erection, especially during sleep, may be further supported by the beneficial physiological effects of erectory inhibition of urination, thus helping to avoid nocturnal enuresis. However, since women have a similar phenomenon called nocturnal clitoral enlargement, prevention of nocturnal enuresis (wetting) may not be the only cause of support.
